Ensign-Bickford Industries, Inc. (EBI) and its subsidiaries are pleased to be able to offer the employee benefits described herein. The following information highlights some of the benefits offered and does not cover all provisions contained in the actual Plans. The full terms and conditions of the Plans are set forth in the Plan Documents and that text will control as to any questions that may arise concerning any of the Plans. The benefit levels, eligibility requirements, terms of coverage and employer/employee contributions may be changed at any time at the discretion of EBI. In addition, EBI reserves the right to terminate the Plans at any time.

Health & Life Insurance coverage
You are eligible for coverage under these plans on the first day of employment if you are a regular full-time employee and work more than 20 hours per week.

Medical Insurance Coverage
Depending on your location, you will have the option of choosing a Health Maintenance Organization (HMO) or a Point of Service (POS) Plan or a Preferred Provider Organization (PPO). The cost for coverage is shared by the employer and by the employee. The employee’s contribution is in the form of a pre-tax payroll deduction.

Dental Insurance Coverage
EBI offers dental coverage through Guardian DentalGuard Preferred Select Plan administered by CoreSource, Inc. The Plan allows you to choose between in-network and out-of-network providers. It provides coverage for diagnostic and preventive services, such as oral examinations, cleanings, and x-rays; basic services, such as filings, extractions, periodontal treatment, anesthesia, and oral surgery; and prosthetic services, such as dentures, bridgework and crowns. The cost for coverage is shared by the employer (75%) and by the employee (25%). The employee’s contribution is in the form of a pre-tax payroll deduction.

Vision Insurance Coverage
Our PPO vision plan, offered through Spectera Vision, allows for low cost coverage for eye exams, frames & lenses or contact lenses. You choose the level of benefits you receive based on the provider you want (either in-network or out-of-network). This coverage is fully paid by the employee in the form of pre-tax payroll deductions.

Life Insurance Coverage
Basic Group Term Life Insurance and Accidental Death and Dismemberment Insurance equal to one times your annual base earnings is provided to eligible employees at no cost to you. You may also be eligible to purchase additional Supplemental Group Term Life Insurance, at one to four times your annual base earnings. Evidence of insurability may be required. Cost is determined by age and is fully paid by you. Voluntary Accidental Death and Dismemberment Insurance is available in increments of $10,000 for employee or employee and family coverage. Cost is fully paid by you. Dependent Term Life Insurance can be elected and is fully paid by you. Three coverage options are available: $5,000 spouse / $2,000 child; $10,000 spouse / $5,000 child; and $15,000 spouse/ $7,500 child.

Flexible Spending Accounts (FSAs)
FSAs let you take full advantage of existing tax laws by allowing you to pay for certain expenses for health care and/or dependent care with tax-free contributions. We offer both a Health Care Account and a Dependent Care Account with the ability to contribute from $130 to $5,000 annually to either or both accounts.

Business Travel Accident Coverage
When traveling on Company business, you are covered for accidental death in the amount of $50,000. This coverage is fully paid by EBI.

Disability Coverage
Our disability plans are set up to provide income during periods of disability when the employee is medically unable to report to work. Coverage is provided at no cost to you.
For short-term disabilities, up to 6 months in duration, we offer a salary continuation program. Coverage varies depending on exempt or non-exempt employment designation or union negotiated benefit levels. Disabilities are approved, managed and monitored by a third party vendor specializing in disability management.
Long-term disability benefits will begin after a 6-month elimination period and usually follows your short-term salary continuation benefits. Benefits are payable at 60% of your monthly salary to a maximum monthly benefit of $7,500. Benefits continue for up to two years when unable to perform your regular duties in your own occupation or up to your Social Security age if totally disabled from any occupation. Long Term disability is provided as fully insured coverage.

401(k) Savings Plan
This Plan has been instituted to encourage and help you build a firm financial foundation for your future retirement goals. It is designed to provide long-term savings with the incentive of the Company matching a portion of your contribution. You are eligible to join the Plan immediately. T. Rowe Price is our plan recordkeeper.
You may elect to contribute up to 50% of your eligible compensation on a pre-tax and/or after-tax basis, subject to the dollar limits set by the IRS each year. If you are at least age 50, the Plan also allows you to contribute “catch-up” contributions for additional savings; also subject to IRS dollar limits.
For every dollar you contribute to the Plan, the Company will contribute $.75 of the first 6% of contributions. You are eligible for the Company match after completion of six months of active service.
Your contributions and the Company contributions are 100% vested immediately.

Pension Plan
Union employees participate in the Plan after one year of Credited Service and attainment of age 21. You become 100% vested after 5 years of Credited Vesting Service. The Plan is fully funded by the Company. Normal retirement is generally age 65. Early retirement with reduced benefits may start as early as age 55. Benefits are paid in the form of a monthly annuity. Full details are included in the Plan’s legal document.

Vacation
Eligibility for vacation time relates to your length of service. Vacation schedules may vary depending on location, union negotiated benefit levels or manager level status. Specific vacation schedule information is available at each location upon request.

Wellness Program
EBI offers their employees a corporate wellness program. The program is designed to offer employees information on various health-related issues in the form of brochures, health-watch bulletins, health fairs, and company-sponsored health programs.

Employee Assistance Program
The Employee Assistance Program (EAP) is a confidential counseling program available to all employees. It provides counseling for employees and their family members with any personal issues such as family or marital difficulties, stress, anxiety, depression, work-life issues, alcoholism, drug abuse or emotional problems. In addition, the EAP can locate resources for eldercare, financial and legal services, support groups and various other needs. An employee’s participation in this program is held in strict confidence. Up to seven counseling sessions with experience, licensed professionals are provided.
